Investigation Summary
On June 19, 1992, Employee #1, along with another mechanic and an electrician, was assigned to change a pull-out capstan, which, like a conveyor belt, pulls a cable though a machine. Because the belt is tight fitting, Employee #1 and the mechanic had the electrician, who was about 300 feet away, jog the machine using the handle of a small sledge hammer while they tried to put the belt on. As the belt began to move, the mechanic fell, causing the belt to move a little faster and Employee #1 to lose control and slip. His finger was caught between the belt and the pulley. He yelled to the electrician to stop the machine, but the electrician could not hear the employee because he was too far away and the noise in the plant was very loud. The tip of Employee #1's right middle finger was amputated before the mechanic could turn off the machine.
