At about 9:30 a.m. on January 6, 1994, Employee #1 was operating a punch press without a point-of-operation device or guard at Raypak, Inc., in Westlake Village, CA. Three fingers on Employee #1's right hand were amputated. He was driven by a Raypak employee to Westlake Medical Center.
