Investigation Summary
At 10:52 a.m. on October 20, 1992, a faulty V-clamp attached to a pool filter unit exploded while under pressure testing. Employee #1 suffered minor abdominal trauma and was treated and released from the Simi Valley Adventist Emergency Room. There was no protection surrounding the unit and the employer said that the clamp may have been overpressurized (12 psi normal). A citation was issued for a violation of 3301(k).
