Employee injured slipping on floor and falling

At approximately 11:15 a.m. on June 12, 1995, Employee #1, who works in the quality control lab, believes that she stepped on a stir stick, a plastic stick about 1/4 in. diameter and about 5 in. long, and slipped and fell; however, she is not sure what happened. Suffering head, back and knee injuries, Employee #1 was taken to Marian Hospital, where she stayed about four to five hours. Normally, stir sticks are not on the floor and if there are, the stick is picked up immediately. The quality control lab's floor is asphalt tile and Employee #1 was wearing rubber-soled shoes. No water was on the floor, which is kept very clean. No safety citation was issued.
