Employee dislocates shoulder when dragged by forklift

At 12:30 p.m. on October 17, 1994, Employee #1 was attached by his safety belt to a forklift when he tripped over a box and fell. The forklift driver, thinking that Employee #1 was disconnected, pulled him over the box, dislocating his shoulder. Employee #1 was taken to Ventura County Medical Center. Citations were issued-- 342(a) and 3664(a)(12).
