Investigation Summary
At 10:32 a.m. on September 29, 1994, Employee #1 was part of a tree trimming crew that had been working at the Surfside III Condominium project, Port Hueneme, CA. He was climbing a coral tree to reach his working position when a branch broke and he fell about 15 ft to the grass. Employee #1 was taken to St. John's Hospital in Oxnard, CA but was not admitted. On October 17, 1994, the admitting clerk at St. John's Hospital ran a records check, but she was not able to verify that Employee #1 had been treated.
