Investigation Summary
At approximately 3:30 p.m. on October 19, 1995, Employee #1 was helping a truck driver connect a trailer to a semi-tractor. He was using a 4 by 4 piece of wood as a wedge between the fifth wheel plate and the trailer. When the driver moved the tractor, wood was thrown up against the trailer edge. Employee #1's finger was on the top side of the wood when the assembly shifted, amputating the tip.
