Employee's head injured when struck against forklift

At approximately 3:00 p.m. on July 27, 1995, Employee #1, of Rodeo's Farm Labor, decided to fix a forklift that was having lifting problems. He climbed atop the forklift to free a hydraulic hose that was caught in one of the chains. The lift fell, causing Employee #1's head to strike against the lift mechanism. He was taken by ambulance to Santa Paula Hospital by ambulance for treatment of head injuries. The employer's representative notified CAL-OSHA at 5:20 p.m. on July 27, 1995. The division responded at 9:00 a.m. on August 4, 1995. A serious citation was issued for the violation of T8CCR 3664(a)(5).
