Investigation Summary
At approximately 6:10 a.m. on July 27, 1995, Employee #1, of Pete Hetting Dairy, fell from the edge of the tub of a hay grinder that was in the process of winding down. His left leg contacted the hammermill and was crushed and later surgically amputated above the knee. Employee #1 was hospitalized as a result of his injuries and received rehabilitation.
