Employee's thumb injured when caught under air cylinder

At approximately 8:30 p.m. on December 5, 1995, Employee #1 was assembling boogie boards at his place of employment. He was installing a device that would act as an anchor for the boogie board's tether by using an air cylinder to marry a male and female part. Employee #1 was placing a male and a female piece together when the two parts slipped and his left thumb became caught under the end of the air cylinder. His thumb was partially amputated and he underwent surgery to repair the damage.
