Employee's toes crushed by falling I-beam

At approximately 11:40 a.m. on August 30, 1995, Employee #1 and a coworker were using an overhead hoist to move a steel I-beam. The 3,000 lb beam slipped from the hook of the hoist and fell approximately 3 feet, striking Employee #1's left foot and crushing his toes. He was hospitalized for two days. The employer was cited for a serious violation of T8CCR 4999(b)(1) because the load being hoisted and maneuvered was not attached to the hook by means of a sling or other suitable means to ensure the safe handling of the load.
