Investigation Summary

At approximately 1:55 p.m. on September 18, 1995, Employee #1, an inmate with the California Department of Corrections, was assigned to top a tree that was infested with beetles. He had been trained by the California Department of Forestry, which had also provided him with all the safety equipment normally used in performing tree work. The safety equipment consisted of gaffs, climbing gear, safety line, chaps, helmet, and gloves. Employee #1 climbed the tree, secured himself with the safety line, and began to cut limbs. He accidentally cut his safety line and fell 50 ft to the ground, breaking both legs and his jaw. Employee #1 required hospitalization.
