Investigation Summary
At 3:45 a.m. on September 1, 1995, Employee #1 was installing a public address system in a large retail store. He was riding in an elevated cage that sat on a platform attached to the tines of a forklift. The platform was securely attached to the tines and it was equipped with a back guard. The forklift was moving when the back guard struck a 10 in. water line, causing the top section of the forklift basket to fall. Employee #1 fell 15 ft and landed on a concrete floor. The employer was cited for a serious violation of T8CCR 3657(h)(8),for moving or driving a forklift with an employee on an elevated platform.
