EMPLOYEE INJURED IN FALL FROM LADDER

At approximately 1:00 p.m. on January 11, 1998, Employee #1 was moving bags of charcoal, each weighing approximately 10 pounds, from an upper shelf to a lower shelf. Employee #1 fell but has no memory of the incident, and his physician says he may never remember. Employee #1 was found on the floor by a customer's child. His head was on the bottom shelf, and the ladder he had been using was on the floor. Employee #1 sustained two hairline fractures to his skull and was hospitalized for two days.
