Employee's fingers amputated after being crushed in rollers

At approximately 5:45 p.m. on January 31, 1996, Employee #1 was cleaning a rubber print plate that was mounted on a steel roller. The plate and rollers were part of the auxiliary equipment used in manufacturing plastic bags. The plate has an imprint of a customer's name and when it picks up ink from the steel rollers, it imprints the name on the plastic bags. Employee #1 was using a rag to wipe the rubber plate when the rag became caught between the ink roller and the rubber plate. His hand was pulled into the rollers and crushed. Employee #1 was taken to a medical facility, where his fingers were surgically amputated.
