Employee injured between cylinder and round-up ring plate

At approximately 11:00 a.m. on August 7, 1995, Employee #1 was fitting a round-up ring plate to a 60 in. i.d. cylinder located in the west end of the south bay roll area. The 60 in. o.d. round-up ring plate was somewhat smaller than the i.d. of the cylinder. When Employee #1 stood atop the ring plate to check the equal gap between the ring plate and cylinder wall, a support clip broke loose. This caused the ring plate to tip over and pin Employee #1's legs between the edge of the ring plate and the inside wall of the cylinder. Employee #1 was hospitalized for two weeks in Loma Linda University Medical Center with serious injuries to his legs, foot, ankle, and hands. A general citation of T8CCR 3328(e) was proposed against the employer for not securing the ring plate on the cylinder before standing on it.
