Investigation Summary
At about 10:45 a.m. on July 25, 1995, Employee #1 was approximately 15 ft above a concrete floor in the basket of a Genie personnel lift, model PLC-24, serial #14-89-18046-B, cleaning a portico ceiling in front of a resort clubhouse entrance. A coworker on the floor was pushing the lift while Employee #1 was in the basket. Outriggers had not been installed on the lift prior to its use. As the coworker pushed the lift to move it, the lift fell over onto its side and Employee #1 fell onto the floor, sustaining a compound fracture of his right elbow and other injuries. The manufacturer's operating instructions specify that outriggers must be installed before the lift is used and that it should not be moved with the basket elevated. Employee #1 was hospitalized at Desert Hospital, Palm Springs. A general citation of T8CCR 3638(d) and a serious citation of T8CCR 3646(b) were issued to the employer.
