Employee's feet crushed under the weight of falling machine

At approximately 12:00 noon on May 24, 1995, Employee #1, the plant manager, and five coworkers were moving a multi-station edge banding machine from the packaging area to the panel department. The 8,000 to 10,000 lb edge banding machine was 20 1/2 ft long and 3 ft 9 1/2 in. high and was loaded on two small wooden dollies. When the six employees attempted to turn the machine at the east end toward the north end, the dollies broke under the machine's weight and it tipped over toward the north side. Employee #1 was steering the machine from the north side when it fell, pinning both his legs and causing serious injuries to his left foot and right ankle. A serious citation of T8CCR 3328(a) was issued to the employer for using substandard dollies.
