Employee's finger amputated between v-belt and pulley

At approximately 7:35 p.m. on June 12, 1995, Employee #1 was working on a trolley sorter jam that occurred at the far east end of a trolley sorter in the shipping area. He was attempting to back up the trolley sorter to clear the jam. Employee #1 climbed an 8 ft fiberglass stepladder to access a Unibilt-20250 motor that was located 10 ft above the floor. He deenergized the motor first, then attempted to manually pull the v-belt backward. His right index finger became caught between the v-belt and pulley and the tip was cut off. He was first taken to Milliken Medical Clinic, then to Chino Medical Center for surgery. The employer had not established training procedures for reversing the trolley sorter or specific instructions on hazards unique to this job assignment. A general citation of T8CCR 3203(a)(7) was issued to the employer.
