Employee's leg fractured in fall with storage rack beam

At approximately 12:15 p.m. on May 1, 1995, Employee #1 and a coworker were installing a fourth-level horizontal beam on the vertical posts of a storage rack structure. They were working from the 2 3/4 in. wide third-level beam. The coworker had set the back end of the beam using a J-bolt safety pin with the vertical post. When Employee #1 attempted to set the front end of the beam with the vertical post by hitting it with a hammer, the beam popped out away from the rack structure. Employee #1 was hanging onto the beam when it came loose from the back end, and he and the beam fell approximately 11 ft 4 in. onto the concrete floor of the warehouse. He sustained a serious fracture to his left leg. Employee #1 was admitted to Loma Linda University Medical Center for surgery and treatment; he was released on May 9, 1995. The employer was cited for a serious violation of T8CCR 1637(a), for lack of fall protection.
