Investigation Summary
At approximately 3:30 p.m. on April 18, 1995, Employee #1, of Sundance Spas, was operating an unguarded table saw. He had already cut approximately 40 pieces of wood when the piece he was cutting started to splinter on the left side of the saw blade after having been cut. The piece flipped back up and landed on his left hand. When the wood hit the back of his left hand, his fingers contacted the saw blade, resulting in amputation.
