Investigation Summary

Investigation Nr: 170591515
Event: 03/27/1995
Mechanic killed when crushed in pickup truck

Employee #1, a heavy duty mechanic for US Borax for 14 years, had just come on duty for the swing shift and was assigned to work on a piece of equipment nearby in the pit. He was descending the #24 stripping road following a 170 ton Electrihaul (#51). The Electrihaul's operator stopped door to door with a loaded Electrihaul (#58) that was coming up grade leaving the pit to talk about a problem with #58. As Electrihaul #58 pulled away, Electrihaul #51 blew its air horn to stop it. Electrihaul #58 stopped, not because of the air horn blast but to speak to Employee #1, who was in the truck behind Electrihaul #51. The driver of Electrihaul #51, assuming that Electrihaul #58 had stopped because of the honking horn, put his truck in reverse and backed over Employee #1 in the pickup truck. He felt the resistance and immediately pulled forward off the pickup. Employee #1 was pinned in the truck. It took almost 1 1/2 hours to extricate him. The coroner's office said that Employee #1 had died almost instantly.
