Investigation Summary
An electric utility employee was standing on a platform on the forks of a Raymond Reach-Lift as his supervisor, who was operating the forklift, lowered the forks to 1 meter above the concrete floor. The supervisor was going to retract the platform when the first employee placed his right hand on top of the extension mechanism. As the forks retracted, the employee's right index finger was caught in a pinch point of the extension mechanism. He was hospitalized for his injuries for at least 2 days.
