EMPLOYEE'S FINGERS CAUGHT IN MACHINE, AMPUTATED

At approximately 6:00 p.m. on June 1,1994, Employee #1, a miscellaneous plant and system operator for Test Laboratories, Incorporated, associated with Brewster Foods, attempted to remove a thin baglike plastic shroud from an air lock. The air lock was associated with a cyclone used in the processing of animal food supplements. The rotating airlock grinder was not de-energized. As Employee #1 was removing the plastic shroud, his left thumb and index finger received injuries which resulted in partial amputation.
