Employee's finger amputated by chain drive of conveyor

At approximately 7:01 p.m. on May 29, 1994, Employee #1, an employee of Olsten Staffing Services, who was working temporarily for AFG Industries, was sweeping and removing broken glass from in and around the glass production area conveyor. When he attempted to remove a piece of broken glass from the conveyor roller, his gloved left hand contacted the unguarded chain and sprocket drive assembly, which amputated his left index finger. The secondary employer was cited for a serious violation of T8CCR 4075(a).
