Investigation Summary
A telephone line worker standing on a fiberglass ladder had installed a telephone service drop to a house. The 28-foot ladder was in good condition and was properly labeled. The ladder was on firm footing, was at its proper incline, and was secured to the telephone line while the employee was making the service drop connections. The employee, who had 18 years' experience, had received training in ladder safety with annual refresher training.
