EMPLOYEE KILLED WHEN RUN OVER BY TRAILER DOLLY

At approximately 10:10 a.m. on May 9, 1994, Employee #1 was removing chain binders that were restraining a lowboy trailer dolly that was to be hoisted from the bed of the lowboy trailer that he had parked previously. There was approximately 18 inches of clearance between his lowboy trailer and another lowboy trailer that was parked beside him. The other lowboy trailer was previously loaded and ready to depart to a staging location. As Employee #1 removed the chain binders, the adjacently parked trailer began to move forward. A witness observed the close proximity and shouted to get the attention of Employee #1. Employee #1 turned around and was immediately struck and run over by the fully loaded 16-tire dolly and was killed.
