Investigation Summary
At approximately 11:00 a.m. on April 1, 1994, Employee #1 was painting an outside wall of a building from an elevated work platform raised to approximately 14 ft above the ground. A truck that had delivered freight to the company exited the doorway where Employee #1 was painting and struck the elevated platform. The platform overturned and Employee #1 fell to the asphalt driveway.
