Investigation Summary
Employee #1 had just cleaned the rollers of a machine and a restarted it to check her work. She saw some debris on the rollers and was reaching in to remove it when her right hand became caught in the rollers. It was drawn in from her fingers to her palm, sustaining severe crushing injuries.
