Investigation Summary
At 9:18 a.m. on October 14, 1994, Employee #1 and two coworkers were removing phone lines from telephone poles on the northeast side of a street. Employee #1 was in an aerial bucket called a Pitman Aerial Device, model HS-42-HAB frame, which was mounted on a 1986 F8000 Ford truck. The phone poles in this area were old and somewhat rotten. After having its lines removed, one pole began to tip over somewhat. The two workers on the ground tried unsuccessfully to push it all the way over. Unwilling to leave the pole in a precarious position, they tied a nylon rope to it and attempted to lift it up using the winch on the boom of the aerial unit. The rope broke, and the reaction ejected Employee #1 from the bucket to the ground. He suffered serious injuries and was taken by paramedics to Western Medical Center in Santa Ana, where he was pronounced dead on arrival. Employee #1 was not using the aerial bucket's required safety belt.
