Investigation Summary
Employee #1 was a metal decking installer who worked for E.G. Metal Corporation. He was straddling a 4-inch structural steel beam, with a stack of metal decking beside him, while cutting the band that was holding the stack together. When the band was cut, the stack shifted and struck him, causing him to fall about 12 ft to the concrete surface below.
