Investigation Summary

Investigation Nr: 170355986
Event: 08/09/1994
EMPLOYEE FRACTRURED BACK DUE TO FALL FROM ROOF

Two employees were removing dry, rotted roof sheathing from the top of a one-story wood frame workshop at an adult school. Because the roof was rotted, the supervisor instructed both employees to lay 0.5-in. plywood across the roof, supported by the roofing timbers. The supervisor further instructed both employees to remain on the plywood work surface to remove the old roof sheathing. While on the roof, Employee #1 stepped off the plywood work surface onto a section of the roof that had rotted roof sheathing. He fell through the rotted sheathing to the concrete floor below. He fractured his back, sustained lacerations, and was hospitalized.
