Investigation Summary
On July 25, 1994, Employee #1 was working on a 9 ft 6 in. high elevated work platform with a hinged section that swings up. The platform is lowered in front of a trailer to allow a "tarper" to check the tarp cover on trucks to insure that it is properly installed and covering the cargo. The tarper walks onto the platform to perform his tarper responsibilities and then walks back onto the platform and raises the hinged section to allow the truck to proceed. When a truck pulled up the driver did not check to see if the hinged section was raised. He pulled forward, hit the hinged section, and knocked over the platform, injuring Employee #1, who suffered fractured ribs, abrasions, a contusion, and lacerations.
