Investigation Summary
At 11:45 a.m. on June 30, 1994, Employee #1, a forklift operator, had just set a box of coiled twine on his forklift beside his seat. (All operators carry a box of coiled twine to stabilize the top layer of cartons.) He was moving his forklift ahead slowly when he noticed that the end of the twine was hanging over the edge of his forklift. He grabbed the twine to pull it up; at the same time, the forklift's right rear wheel ran over the end of the twine. Apparently his right index finger was caught in a loop of twine. It was severed at the first joint. Employee #1 and his fingertip were taken to the UCLA Medical Center. The staff treated his amputation, but was unable to reattach his fingertip.
