Investigation Summary
At approximately 3:30 p.m. on June 24, 1994, Employee #1 was putting ravioli into boxes on a conveyor. There were two conveyor lines joined end to end with separate prime movers. The first conveyor is where ravioli is put into boxes. The filled boxes then run to the second conveyor with a speed of about 100 ft/min. The middle finger on the right hand of Employee #1 was caught between the moving conveyor and the stationary rails causing it to be amputated. She was taken to Golden West Medical for treatment. Employee #1 said that she did not know how here finger got caught between the moving conveyor and the stationary rail.
