Investigation Summary
Employee #1 was using a long pole with a knife attached to the end to try to cut across a sheet of cardboard. The machine was still in operation and his arm was pinned between the long pole and the paper sheet and pulled into the approximately 340 degree F rolls. He suffered third-degree burns on his left arm and hand.
