Investigation Summary
Employee #1 was operating a Bliss pneumatic power press in the inch mode because an air line to the single stroke mechanism had developed a leak. While Employee #1 was removing a completed piece of work from the press, a stroke was initiated and caught his fingers in the point of operation. The left index finger was amputated at the second knuckle and three other left fingers were completely amputated.
