Investigation Summary

At about 4:00 p.m. on May 6, 1994, Employee #1 and another press operator were installing a large printing plate on the dye cylinder when Employee #1's left hand finger tips were caught between the ink transfer cylinder and the print cylinder. He was taken to a local doctor for treatment. He received partial amputations of the finger tips, but the amputations did not involve the bones. The main power source of the press was de-energized and lockout/tagout procedures were followed. The press has a separate power source used for jogging the print cylinder only. The print solid plate was 45 inches wide and 65 inches long that was installed on a cylinder 50 inches in circumference and 80 inches long. It was heavy, requiring the use of both hands. When the press operator jogged the cylinder, Employee #1 forgot that his left hand was very close to the nip point of the inking cylinder and print cylinder. This was not a serious injury. The employer does have a written lockout/tagout procedure, which were followed by the employees during the set up operations. No violation was noted. As a result, the employer will re-instruct the two employees on the hazards and accident preventive measures during the set up operations.
