Investigation Summary
At about 1:45 p.m. on April 19, 1994, Employee #1 was leveling a pepperoni in a box that came out of a discharge chute to the pepperoni dicer with his right hand while the machine was running. His right hand went inside the chute, and the cutting knives amputated his thumb and lacerated the rest of his fingers. He was taken to a local hospital. The discharge chute was about 10-inches inside the box, as measured from the top of the box to the bottom of the discharge chute. The accident occurred when the cut pepperoni filled up the box in a cone shape and started to block the chute opening. Employee #1 began to level the pepperoni with his right hand and somehow went inside the discharge chute where his hand came in contact with the cutting knives. The discharge chute did not have any guards to protect against accidental contact with the revolving cutting knives. The injured employee stated that he did not shut-off the machine and execute the lockout procedures, because he was not cleaning or adjusting the machine. He received training on lockout procedures. The primary cause of the incident was the lack of guards to prevent accidental contact with the cutting knives. The employer was given a serious citation for violating Section 8 CCR 4553. The violation was abated by installing metal strips not crossed at the chute opening.
