Employee's hand injured when caught in moving machinery

Employee #1, a maintenance mechanic for Setco, Inc., had opened a panel to an injection molding machine to observe an ongoing problem with a machine part. He slipped on a small plastic pellet on the floor and his left arm entered the moving parts. Employee #1 suffered an open fracture of his left wrist and a tendon laceration.
