Employee's finger amputated when caught in press rollers

At approximately 8:00 a.m. on August 22, 1995, Employee #1, of I. L. Walker, Inc., Los Angeles, CA, a manufacturer of folded paperboard cartons, was operating a Komori 40 #1 printing press. He had been hired on July 5, 1995, and had not yet been given any safety training. He had approximately 20 years experience operating Komori 40 presses with other employers. With this employer, his regular assignment was operating a Roland 800 press. On this day, his duties included checking the cleanliness of the Komoii 40 press rollers because the Roland 800 press was down. The rollers were in motion, guarded by location. He reached into the rollers and touched them with his right index finger. Four of his fingers were drawn in, and his middle finger was partially amputated. He was taken to the White Memorial Hospital, Los Angeles, CA, where he was treated and released the next day. Causal factors include touching moving parts in machinery. A general violation, 8CCR 3203(e), was observed, cited, and abated.
