Investigation Summary
Employee #1, an inspector, was operating a Pernick textile inspection machine when he went to repair a coworker's similar machine. Employee #1 opened the side cabinet covering the machine's chain drive unit to find the problem without locking out the machine. The enclosure had not been locked because with the cabinet kept open, air cooled the computer inside, preventing it from overheating. The problem had occurred when the key in the machine's drive shaft had dropped off, unlocking the chain sprocket drive from the shaft. When Employee #1 attempted to reinstall the key, his right little finger became caught in the nip point of the sprocket and the end of his finger was amputated. Employee #1 was not part of the company's maintenance department.
