Investigation Summary
Employee #1 was feeding by hand potatoes into working on a Butcher Boy meat grinder equipped with a 5 HP motor that the employer used as a potato grinder. The feed throat to the top of the screw mechanism that performs the actual grinding had a 4 in. by 6 in. opening and no point of operation guard. Employee #1's arm was amputated below the elbow when he stuck it through the hole into the point of operation. Employee #1 had not been trained prior to the assignment and had only been working on the machine for 15 minutes. He had been a regular employee of this food production company for three months.
