Employee's fingers injured in bag machine

At approximately 7:27 a.m. on June 9, 1995, Employee #1, of Elkay Plastics Co., Vernon, CA, was repairing flat belts under an operating bag machine. His left hand became caught between the belts and the rollers and he sustained lacerations and fractures to his third, fourth, and fifth fingers. Employee #1 was taken to Santa Marta Hospital, where he remained for three days and was later treated as an outpatient. He did not use any lockout procedure prior to beginning repairs.
