Investigation Summary

Investigation Nr: 170882294
Event: 08/26/1994
Employee burned by gas explosion

An oil well service company was changing over an old oil well as part of an abandonment. The changeover involved pumping slurry into the well to displace any residual oil or gas. The residual oil and gas were not being forced out through a hose into a Baker tank, but were spilling out into the well cellar. Employee #1 was nearby pumping residuals from the cellar into a vacuum tank mounted on a truck and powered by a diesel motor. The area filled with gas forced out of the well, which was then ignited by the vacuum pump motor. The resulting flash explosion seriously burned Employee #1.
