Employee sustains 5 in. face laceration when saw kicks back

Employee #1 was lying on his stomach under the foundation of a house, preparing to cut a piece of 3/4 in. plywood off the foundation base. He had a 4 1/2 in. Makita saw (115 volt, 60 hz, serial #158068, model #95148, 4.6 amp., 1,100 rpm) in his left hand and was supporting it with his right hand. He had started cutting at the base of a 2 ft wide by 8 ft long by 3/4 in. thick piece of plywood and was cutting vertically when the saw kicked back. The unguarded portion of the blade lacerated his face from the bottom of his left eye to his lip, approximately 5 in. Employee #1 was hospitalized for stitches.
