Employee injured when struck by flying pieces of metal

Employee #1, who works for a company that manufactures furnaces, was preparing his lunch while standing against a table on the southeast wall of a welding shed. Suddenly, coworkers heard Employee #1 scream for help and saw him stagger toward the shed entrance, then collapse on the floor, with his left leg bleeding. No one can recall what process was active or what equipment was involved. Employee #1 suffered several wounds from metal pieces that had become embedded in his thigh. The investigation did not uncover any cause for the accident. Employee #1 could not be located and very little information was gathered from the one available witness.
