Investigation Summary
Employee #1, age 20, was an experienced machine operator with the Allied Record Company, a manufacturer of CDs and records for the recording industry. He was setting up four silk screens for a run on a Kamman printer, checking to ensure that all of them were clean and devoid of holes. He was apparently doing this with the machine running when his little finger was pulled under the housing for an ultraviolet lamp. He suffered a fractured finger and ligament damage, and was taken to Santa Marta Hospital for surgery. There were no witnesses to the accident. Employee #1 had been trained in machine shutdown, and guard removal while the machine was still running was contrary to his employer's written, verbal, and posted rules. This accident was not the result of a lockout/blockout violation because the normal procedure for production of compact discs requires that a guard be removed during the initial set-up. The employer was cited for lack of guarding from inadvertent contact with moving parts.
