Investigation Summary
On May 11, 1994, Employee #1 was driving a Clark ECS25 Model Number 357-0085-6517 forklift outside near the side of the food plant. He was not carrying any loads but he was operating the industrial truck very close to the safety posts on the side of the plant. Employee #1 was not looking in the direction of travel when he attempted to adjust the forks. He was moving approximately 6 miles per hour. He suddenly struck a safety post near a "Skippy" peanut butter container that was approximately 12 feet high. The impact caused Employee #1 to thrust forward and his head struck the rollover bars. He was not wearing the seat belt provided in the fork lift. After striking the support, the forklift remained upright. Employee #1 was found face down on the ground and conscious approximately 8 feet from the impact point. Employee #1 sustained a serious neck injury which resulted in temporary and possibly permanent paralysis. He also suffered lacerations and bruises.
