Investigation Summary
Employee #1, of La Vista Products, Inc., was cleaning a large (5,000 lb capacity) ribbon blender used for mixing flour. The cleaning procedure was to lock out the power to the blender, and then to enter the blender and wipe or sweep the flour into the chute of a small auger conveyor at one end. With the blender locked out, this conveyor could be turned on safely from a motor controller mounted outside the blender. Without the blender locked out, this same motor controller turned on both the conveyor and the blender. Employee #1 entered the blender to clean it without locking out the blender at the circuit breaker. When he called to a coworker to turn the conveyor on, the blender blades began to drag him around the blender hopper and severely lacerated his head, body, and one leg, before the coworker turned it off. Employee #1 was hospitalized, but survived.
